You can enroll in the program at no cost to you
Enrolling in Madrigal Patient Support gives you access to a dedicated team and a full range of support services. You or your doctor can enroll you in the program when you start Rezdiffra.
Ways to enroll
At the doctor’s office
Your doctor can start your enrollment in the program. To do so, they will ask you to fill out a Patient Authorization form. Signing this form allows your doctor to share your information with Madrigal Patient Support. This information is used to provide treatment access support. To begin your treatment fulfillment process through Madrigal Patient Support, your signature is required.
Enroll online
If you aren’t at your doctor’s office, you can provide authorization for assistance from Madrigal Patient Support electronically. You may need to submit additional documents if you wish to apply for the Bridge or Patient Assistance Programs.
After you enroll, your dedicated Case Manager will call within a few days to welcome you. They will provide an overview of the program, explain your insurance coverage, discuss potential financial assistance options, and answer any questions to help you get started.
