How do I enroll in the Madrigal Patient Support program?
Why do I need to sign a Patient Authorization form?
If your doctor begins your enrollment in the program, they will ask you to fill out a Patient Authorization form. Your signature on this form gives your doctor permission to share your information with Madrigal Patient Support. This information will be used to provide you with treatment access support. To begin your treatment fulfillment process through Madrigal Patient Support, you must sign this form.
