How do I enroll in the Madrigal Patient Support program?

You or your doctor can enroll you in the program when you start Rezdiffra. If your doctor begins your enrollment, you will need to sign a form. If you decide to enroll outside of your doctor’s office, you can provide authorization for assistance from Madrigal Patient Support electronically. Learn more about how to enroll

Why do I need to sign a Patient Authorization form?

If your doctor begins your enrollment in the program, they will ask you to fill out a Patient Authorization form. Your signature on this form gives your doctor permission to share your information with Madrigal Patient Support. This information will be used to provide you with treatment access support. To begin your treatment fulfillment process through Madrigal Patient Support, you must sign this form.

Where can I learn more about my condition or medicine?

Our Nurse Navigator team provides education about your condition and your prescribed medicine. Your Nurse Navigator can explain how it works, what to expect, and answer any questions you might have. Keep in mind: Your doctor is the best source of information for questions about your health.
